Hearts for Homes seeks to improve the living conditions of low-income senior homeowners, age 60 and older, in Denton County, TX. Core programs include Home Repair, which provides no-cost home repairs; Case Management to review client applications and check in on clients regularly; and Christmas Baskets, which offers baskets of non-perishable food items and durable goods to clients for the holidays. Funds are needed for general operating costs in order to grow to meet demand and sustain our programs to serve an increasing senior population.

The James L. West Center for Dementia Care serves persons with dementia disorders and their families in a compassionate and spiritual environment. Since , the James L. West Center for Dementia Care has been dedicated to providing individualized care and support to enhance the quality and dignity of each person's life throughout the course of the disease. More than 24 education and healthcare organizations use the West Center as their clinical class site to learn about the complexities of aging.

Funds will provide operating support for the Center's dementia-specific Senior Adult Day Care, financial aid program, and education classes. Juliette Fowler Communities provides the dignity of choice, builds community, and creates meaningful connections for the children, youth, and elders that they serve. Juliette Fowler Communities includes independent and assisted living, health and rehabilitation services, memory support, a foster care and adoption program, and The Ebby House for young women who have aged out of foster care or are at risk of homelessness due to neglect, abuse or abandonment.

The mission of the Wellness Center for Older Adults WCOA is to assist older adults achieve their highest level of physical, mental and spiritual well-being. Funding will support program staff salaries and operating expenses for all three programs allowing them to provide services to seniors at little to no cost.

Meals on Wheels Senior Services helps keep the elderly living in their homes as long as safely possible through nutritious meals, daily human contact, and supportive services. With Rockwall being one of the fastest-growing counties in Texas and the increase of older adults in the county, there is a high demand for supportive services needed to maintain their independence.

Funding will support the cost of meals for the Nutrition Program for elderly and disabled citizens. Meals-On-Wheels Inc of Tarrant County's mission is to promote dignity and independence of homebound elderly and disabled persons by delivering nutritious meals and supportive services. Core offerings include the Home-Delivered Meals program which provides professional case management and fresh nutritious meals to homebound elderly and disabled residents of Tarrant County each weekday; and the Client Assistance program which offers supportive services to meal clients.

They provided , meals last fiscal year to 4, unduplicated clients and made 11, in-home case management visits. Funds are requested to provide meals and supportive services to clients, and purchase a new Rational Combi-Oven for meal preparation. Mid-Cities Care Corps' mission is working together to preserve the independence and well being of senior neighbors in Northeast Tarrant County.

Programs include Transportation, which involves driving seniors to medical appointments; Helping Hands, which involves assisting seniors with minor home repairs, building wheel-chair ramps, installing grab-bars, adopt-a-lawn projects, and group projects; and 3 Social Outreach, which prevents isolation among seniors with social events. In , Mid-Cities expanded Helping Hands and Social Outreach programs, which is a reflection of collaborative efforts with community partners.

Funds are requested to support all three of their programs. Seniors' Pet Assistance Network's SPAN mission is to secure joyful, healthy relationships between low-income seniors and their companion animals.

SPAN provides pet food and supplies to clients and pays for basic veterinary services, including annual exams, tests, and preventatives for heartworms and fleas to increase their service and impact. In the last year, SPAN hired their first employee, a part-time Executive Director, and updated their logo and mission statement.

Funds will support the purchase of pet food and supplies and pay for veterinary visits. The Senior Source seeks to improve the overall quality of life and empower all older adults in greater Dallas to thrive. Core programs include the Elder Financial Safety Center, which provides integrated economic stability services; Caregiver Support Program, which addresses the needs of caregivers of older adults; and Long Term Care Ombudsman Program, which advocates for the rights of residents in long-term care facilities.

Funds are needed to support our services to low-income older adults and our services to caregivers of the elderly. The mission of Tyler Street Tower is to provide safe, affordable housing and social services to lower-income older adults. This program provides a multi-faceted system of support services for family caregivers and grandparents raising grandchildren or older individuals who are relative caregivers. The intent is to ease the emotional, physical, and financial strain of caregiving.

Information Benefits Counseling Provides. The Benefits Counselors are specialists trained in Medicare eligibility, benefits and options, and health insurance. They provide objective and free assistance so older adults can make informed choices about their benefits.
